This policy explains how My Diabetes Circle collects and uses information during its early beta. We may update it as the product and our practices develop.

Information we collect

We collect information you submit in a beta application, such as your name, contact information, whether you manage your own diabetes or help someone else and general information about your care-coordination needs.

If you receive beta access, the app stores the account and care-coordination information you choose to enter, which may include supply information, appointments, care notes, medications, providers, emergency contacts and check-ins.

How we use information

We use this information to review applications, provide and improve the beta, communicate with participants, troubleshoot problems and protect the service. We do not sell personal information.

Who can access it

People you invite to your circle can access the information shared within that circle according to their assigned access. Other beta participants cannot view your circle. Authorized My Diabetes Circle administrators and service providers may access information when necessary to operate, secure, troubleshoot or improve the service.

Beta application forms

Our website currently uses a third-party form delivery service to send beta applications to My Diabetes Circle. Please do not submit Social Security numbers, financial account information, insurance identification numbers, passwords or detailed medical records through the application form.

Data choices

You may ask us to correct or delete beta information by emailing mydiabetescircle@gmail.com. We may retain limited information when reasonably necessary for security, legal compliance or resolving disputes.

Important limitation

My Diabetes Circle is a care-coordination tool for individuals and families. It is not an electronic medical record, emergency service or substitute for professional medical advice. Do not use the service as the sole location for information needed in an emergency.
